Pupil Absence Forms (Sickness and Holidays)
Please complete the forms below to provide written confirmation of the reasons for a previous absence (for example, illness) or to inform school about any planned absence (for example, holidays during term time).
Please note that we will not authorise requests for holidays or time off during term time, except in exceptional circumstances. The Board of Governors have agreed that the definition of an 'exceptional circumstance' is that the event cannot reasonably take place during the school holidays. The Headteacher will use his professional discretion and judgement in deciding whether the request meets this criterion.
